Thanks for your help guys, I ran a fs repair on the mounted partition this
morning (eeek!) and it had solved the issue! :-) I made sure to stop any
processes which were likely to be writing to the FS during the repair to try
and minimize the risk, seems to have paid off!
I'll keep an eye on this system and hopefully it'll stay alive.

Looks like file system checks during boot have all been disabled in
voyage. Wasn't quite aware of that myself. But it's better anyway,
cause on errors the system might drop you into a shell to which you
wouldn't have any remote access.
You can find out when a file system was last checked yourself btw, just
look at the output of tune2fs -l /dev/hda1
Well then, run the check while the system is up and running. Just make
sure the root fs is mounted read only, which it is be default (else
remountro):
e2fsck /dev/hda1
